TruthGirlz Permission Form

Please submit this form for your daughter if you give your permission for her to help at PRC Life Banquet on May 5th.

They would need to arrive at the Civic Center by 5pm. We will provide pizza for dinner.

She will earn a TruthGirlz shirt if she doens’t already have one for helping at this event.

They will need to wear black pants and comfortable shoes.

Pickup time is between 8:30pm and 9:00pm.
